All ASU applicants are required to submit the following:

• Properly completed and signed application form (hardcopy or electronic)
• $30.00 non-refundable application fee
  (All applicants are required to pay the $30.00 application fee. Former students who attended another institution after enrollment at ASU are required to pay the application fee. Only former ASU students who have NOT attended another institution after enrollment at ASU are exempt from the application fee. Former students who have not enrolled at another institution are required to contact the Registrar's Office to reapply).

  **This fee must also be paid when an applicant changes their enrollment term.

• Complete Immunization Form.

Two ways to apply:

1. Online.
On-line applications are for undergraduates students only at this time. Have your credit card or checking account information ready. All on-line applicants are required to pay the $30 application fee, including former ASU students.

2. Print a hardcopy application and mail it in.
To use this option, you must have the software Adobe Acrobat Reader loaded on your computer and configured to work with your Web browser (Netscape, Internet Explorer, etc.). The software is available for free by clicking here.
