Lab Forms

Criminal Background Check - Students who have not completed a background check in the past twelve months will need one (so you might wish to bring a completed form to class). If you are unsure when your last background check was run, your instructor can confirm whether or not you need a new one.

The following forms should be printed out for the students' reference and use:

Dress Code and Appearance
Lab Evaluation (completed and returned to the professor at the end of the semester)
Time Documentation (completed and returned to the professor at the end of the semester)
