Using e-Forms

There are a variety of forms available on line. Some are PDF files and others are WEB pages. To view a PDF document, you must have the software, Adobe Acrobat Reader, installed on your computer. All ASU campus computers have this software installed. If you need Acrobat for a home computer, download Acrobat from the internet by clicking on this link. . Download Adobe Reader

Types of Forms

View & Print - This form is VIEWED ON-LINE and PRINTED. The printed form would then be completed "by hand" and then delivered/submitted to the appropriate person.

On-line & Print- This form may be COMPLETED ON-LINE and PRINTED. The printed form is then delivered/submitted to the appropriate person.

On-line & Local E-mail - This form is COMPLETED ON-LINE and the completed form is E-MAILED to the appropriate person. This process does NOT work with WEB BASED E-MAIL (such as Campus Pipeline).

When using Web Based mail, complete the form on-line and then print a copy to submit.

Web Submit - This form is a web page that is completed on line and submitted. It does not require any particular email package.

Keeping a completed copy for your records - Three (3) ways to keep a copy for yourself:

  1. Print the form before you email or submit it.
  2. Save a copy of the completed form to your computer before you submit it. (The save feature is only available if you are using the STANDARD or PROFESSIONAL version of the Acrobat software which is installed on most campus faculty & staff computers)
  3. Email the form to yourself (if NOT using web based mail)
