Tax Forms
A ll new employees should complete and sign the Federal W-4 and State G-4 tax forms when they start work. The forms will be effective with the first wage payment (paycheck). If you do not provide Human Resources with a completed W-4 and G-4 form, taxes will be withheld at the maximum tax rate. Non-resident aliens may be subject to other requirements. Please contact the Human Resources Office with specific questions.
Please complete the Federal W-4 (I-9) and the State G-4 forms.
