Faculty Evaluation Instructions |
If you are administering an evaluation developed by your department that does not require scanning by Information Technology Services, disregard these instructions.
- All departments will receive a packet that contains labels for each course. Each label contains the following information: Instructor's Name, Department, Course Description ( i.e. ACCT2101A ) and course enrollment. These labels will be generated by Information Technology Services and distributed to the departments three (3) weeks after classes begin. A sample label is shown below. As of Spring Term 2004, IT IS NO LONGER NECESSARY TO BUBBLE IN THE SPECIAL 10 DIGIT CODE NUMBER ON THE ANSWER SHEET.

INTRODUCTION TO Information Technology Services I
- The student should write in the Course, Dept #, Section, Instructor and Term on the answer sheet.
- A separate manila envelope should be prepared for each course label.
- Insert blank Scantron answer sheets into the envelope according to the number of students enrolled in the course.
- Affix the label in the upper left hand corner of the envelope.
- DO NOT include any handwritten comment sheets in the envelope with the completed Scantron forms.
- As evaluations are completed and turned in, you may forward them to Information Technology Services for scanning.
- The deadline for returning the completed evaluations to Information Technology Services is the day after the last day of classes for the term being evaluated.
- The Augusta State University Evaluation/Survey Scantron form is available from the ASU Central Supply Office.
- The Official Course and Instructor Evaluation tool is available from your Dean.
