Request for facility work need to be submitted to the Physical Plant using a Work Request Form, or calling in a service call to 1590.

WORK REQUEST FORM
The Work Request Form is used to request repairs, alterations, and modifications to campus facilities that do not require immediate attention, nor are extensive in scope. Examples include: painting, new/replacement keys, carpenter shop fabrications, parking lot stripping, electrical lighting changes, and ventilation modifications. Special events setups are also requested using this form. Forms need to be submitted a minimum of ten (10) calendar days prior to the needed date.

This form is currently available as a hard copy, multi-copy form or as an E-Form.

SERVICE CALL (X1590)
Phone calls are used to initiate small projects that need immediate attention. Examples are heating, cooling problems, power / light failures, chemical spills, safety hazards, roof leadks, and problems with restroom fixtures. Response to service call is normally under four (4) hours.
