Directions for Ed.S., M.Ed., or M.A.T. certification paperwork

In order to make changes to an existing certificate, or to apply for a new certificate, the Professional Standards Commission requires official documentation. The state mandated PSC certification application is available at this website:

Print out the application, and fill out according to the directions listed below. It is very important that you follow the directions closely. The completed certification application must be returned to Julie Gray , certification officer, by 4:30 on Tuesday, June 9 . Failure to return the completed certification paperwork by June 9 will result in a delay in the processing of your paperwork.

Please remember that all degrees for summer semester will be awarded on August 5, 2009. Because summer semester is divided into different terms, it is possible that your coursework will be completed before the end of July. However, certification paperwork can not be processed until after the degree has been awarded.

On August 10, 2009, the complete certification application packet will be mailed to your home. This packet will contain the PSC certification paperwork and the official ASU transcripts. Take the UNOPENED envelope to your county personnel office for processing. If you are not currently employed in a Georgia public school system, the certification paperwork will be sent directly to the Georgia Professional Standards Commission. If you work in a school system in South Carolina , you must contact me to fill out additional certification paperwork. You do not need to contact the ASU Registrar's office to request your transcripts, the transcripts will automatically be sent to my office after graduation.

In addition to the PSC certification application, you will need to include a copy of your certification record which is available on the PSC website: https://www.gapsc.com/certification/lookup.asp . This must be printed in Landscape view so that all of the information will be printed on the page.

In order to access your records, type in your SSN, then print out the certification record. If you are applying for your first Georgia certification, you will not have a PSC record.

Georgia PSC Approved Program Recommendation Form

First section:
•  Your demographic information.

Certification official section:
•  Leave blank

Box titled “Georgia Program Use only”:
•  The only information that you need to fill out is the date that you passed the Praxis II/GACE content assessment.

Box titled: Certification/Endorsement fields:
•  Write in the name of your ASU major and the field code. See page 2 of the Approved Program Recommendation Form for a list of fields and field codes. (next page)

**** If you are completing the EDS in Teaching and Learning, the certification field is the field in which you took your content courses in the EDS program.

Program Completion Date:
•  If you have not passed the GACE Content Assessment by June 9, 2009, leave the program completion date blank.
(You must contact me when you do receive the passing scores. The GACE Content Assessment scores are not sent directly to my office.)
•  If you have passed the GACE Content Assessment prior to June 9, 2009, put the date that you passed.

Degree Level:
•  Master's degree – level 5
•  EDS degree – level 6

GEORGIA PSC CERTIFICATION APPLICATION

Section 1:
•  Your demographic information.

Section 2:
•  List the name of the GEORGIA public/private school where you are employed.
•  If you are not employed in a GEORGIA public/private school – leave blank.

Section 3:
•  leave blank

Fee submitted:
•  If you are currently employed by a GEORGIA public school system, there is no charge.
•  If this is your first GEORGIA certificate, there is no charge.
•  If you are not in one of the above categories, there will be a $20.00 charge.

Personal Affirmation:
•  Read the directions and answer each question yes or no. “False statements made in this application may constitute sufficient grounds to take action against, revoke or deny a certificate and may constitute grounds for legal action. Failure to complete this block will result in your application being returned without processing.”

•  Sign and date the form.

Academic Record:
•  If you currently have certification in Georgia, the only colleges that you are required to list are the colleges for the current degree. If this certification application will be your first Georgia certificate, then you will need to list all colleges or universities you attended previously.
•  You do not need to contact the ASU Registrar's office to request the ASU transcripts. The transcripts will automatically be sent to the certification office after the degrees are awarded.

Do you currently hold NBPTS certification? answer yes or no (answer yes if you have National Board Certification)

Assessment Record

•  State/Jurisdiction - the name of the state where you took the Praxis II/GACE Content Assessment.
•  Content/Subject Matter Assessment:
The name of the test, (i.e., Praxis II – Early Childhood or GACE Content Assessment – Middle Grades)
•  Date Passed - if you have not taken the GACE Content Assessment, leave blank

•  Copy attached - leave blank
•  Copy being sent - leave blank
•  if PSC has already received your Praxis II/GACE Content Assessment scores, write the words: ON FILE.
