• Packet Cover Letter W/Checklist (additional necessary items, such as transcripts, will be listed here)
• Faculty Application Form (print, then complete)
• Employment Eligibility Verification Form (I-9)
• Pre-Appointment Personal Data Form
• Security Questionnaire/Loyalty Oath
• Criminal Background Check Form
• Departmental Information Sheet

Additional paperwork will need to be filled out in personnel after you are hired.
