Physical Disability Documentation Requirements

In order for a student to receive course and testing accommodations based on a physical disorder, Testing and Disability Services must receive the following documentation:

A written statement by an appropriate physician which includes the following items:

A definitive diagnosis
The nature of the functional impairment
The length and severity of the impairment
How the disability currently manifests in substantial limitations.
Include suggestions for appropriate accommodations

This is the criteria established by the Georgia Board of Regents.
